Transition – Smooth or Terrifying? It’s YOUR choice!
Many families find the thought of transitioning from school to adult services overwhelming and scary….but it doesn’t have to be! The process of addressing the issues relating to a student’s transition into the world of adult services requires time, thought and planning. When a proactive, well organized, and collaborative approach is taken to the process, a student’s transition can be a smooth, easy paddle rather than a stressful plunge off the edge of a cliff.
These classes will provide the knowledge and empowerment to approach your school district about the services needed to plan a smooth ride into adulthood. There will be two sessions – one for students who are eligible for the Medicaid HCBS Waiver, and one for students who are not eligible.

Topics to be covered include: Person Centered Planning, Circles of Support, School Supports, Community Networking, Educational Connections, Employment Connections, and Housing Options. The HCBS Waiver Eligible Sessions will also cover services available through the waiver. There will be ample time allocated to get both general and specific questions answered.
Scholarships available for all of our programs through the Richard and Nancy Dorschel Family Foundation Scholarship Program. You will be prompted on the online form, to complete necessary information.
Meet the Facilitators:
Cheri Greenauer – Transition Specialist
Cheri has almost 30 years of personal and professional experience in the field of Developmental Disabilities. Her primary area of expertise is transition planning and execution. As a Medicaid Service Coordinator and CSS Broker, she has a working knowledge of all OPWDD transition requirements. She has a fundamental belief in the right of all people to make choices that will allow them to live their lives as they wish, and is well known for her “out of the box” thinking. Cheri has a BS in Sociology from SUNY Fredonia, and has numerous hours of professional development in transition planning.
Laura L. Khedarian – Person Centered Planning Facilitator
Laura is a professional with many community connections, as well as extensive experience in team building, meeting facilitation and creative problem solving. She runs highly effective meetings where participants feel their time has been wisely invested, and makes sure that things are well-documented and action items are followed up on. She can help you to move forward and create the life you want to live. She can help you define a circle of support, a person centered plan, and a CSS plan (if needed) that will help you to move forward and create the life you want to live. She does this with a heart that knows where you are coming from – she has an adult son with developmental disabilities and knows first-hand the challenges associated with building teams of diverse players and developing individualized and person centered solutions.
